Invoices, statements, purchase orders, sales orders, and general purpose forms are available in two different series or classes. The Standard series are printed with blue laser-screening for the body of the form and black for the company imprint. The Economy series have only black outlines for the areas of the form and black for the company imprint. Additional copies feature clear, sharp, carbonless images of the first part. If enhancing your company's image is important to you, our Select series forms feature multi-color forms options.

Custom Designed Forms

This option is perfect if you need specialized forms or have modified software. Custom forms are available in single part laser or up to four parts continuous formats. Simply provide us with any camera ready art needed (if any) and a sample of the layout that you want to use and we will do the rest of the work for you. With our design assistance you can obtain a custom designed invoice or general purpose form that is just what you want it to be. If you need additional features or options not covered below, call us. We can duplicate your existing forms or produce them to your exact specifications.

Laser Form Design Guidelines

Custom designed laser forms in 8 1/2" X 11" formats are available in quantities as low as 250. Your forms will be imprinted in crisp black for Standard Quality Forms or in any one or two PMS colors of your choice for Select Forms.

Continuous Form Design Guidelines

Choose from one to four parts. Part One will be imprinted in crisp black for Standard Quality Forms or in any one or two PMS colors of your choice for Select Forms. Internal copies are sharp carbonless impressions of the first part. Forms are available in 9 1/2" X 7" and 9 1/2" X 11" formats.

Arabic Consecutive Numbers are printed in RED or BLACK ink at factory option.


Starting Numbers should be specified when you order.


Numbered Forms: Blank or imprinted forms may be numbered for an additional charge ranging from $7.50 for 250 forms up to $99.00 for 10,000 forms.

Camera Ready art must be submitted as clean, sharp, exact size black and white original print. Fine lines or type, screens, and heavy solids will not reproduce well. Enlargements or reductions can't be accepted. All artwork is subject to factory acceptance and will not be returned unless requested at the time you order.

Forms Instructions

When available please submit a printed sample of your current forms to specify copy position.

The following options can be included on your checks or forms for an additional $2.50 per line:

• Additional line of type (over 5) in the heading imprint area.

The following options can be included on your forms for an additional $15.00 per order:

• Up to 5 lines of type in body of form. (+$2.50 per additional line thereafter)
• Use of customer supplied camera-ready art or logo.
• Proofs Submitted For Review - (Production and delivery schedules are extended.)
• Imprinting in Non-Standard Positions

You may also choose from 23 other typestyles for imprinted information on your forms. A faxed stylesheet displaying each typestyle may be requested by e-mail, fax or telephone. The stylesheet can be e-mailed as an attached file or faxed directly to you.

We will contact you, if we need additional information such as paper colors or starting check numbers. No fax is needed for repeat orders of checks or other forms that have had no changes since your previous order. You may also e-mail us directly at bizforms@adelphia.net.

We will e-mail or fax you a confirmation on your order and request that you return e-mail or return fax your acceptance of the order. This is the time to review all of the information entered on the order for accuracy and completeness. Your order will be processed and faxed to the printer within 1 working day (Monday through Friday) after receipt of your order acceptance and acceptable form of payment. Our printers normally ship Standard and Economy orders within 2 to 3 working days. Select orders may take from 3 to 10 working days depending on the amount of design work and typesetting required. Camera ready artwork is always forwarded to printers by Priority Mail at no additional charge, but you should allow an extra 2 to 3 working days, when camera ready artwork is required.

Shipping & Handling

UPS ground delivery normally takes from 2 to 3 days from time of shipment in most parts of the country. All orders are shipped by UPS Ground unless you request 2nd Day or Next Day UPS shipments. We recommend 2nd Day delivery for Alaska, Hawaii and foreign or overseas shipment destinations. Shipping is quoted and charged at our true cost. A $2 handling charge is added per order to cover the cost for faxing confirmations and for faxing your orders and sample forms to the printer. There is no 2nd or 3rd handling charge when more than one form or check is ordered at the same time. Payment of customs duties, value added taxes and other fees levied by government agencies on overseas orders are the responsibility of the customer.

We accept Company Checks, Personal Checks, American Express, Discover, MasterCard and VISA as acceptable forms of payment on all orders. Sorry, but we can't accept COD orders. Payment by credit card assures you of the fastest possible delivery time. Check payments on overseas orders must be by certified check or international money order denominated in U.S. funds.
